I bet most of the doctors in Two itself are retired PK medics. They are barely 40 after their term is up and they don't have much else to keep them busy, so it makes sense that they'd move on to a cushy job in district, either training new PK doctors or treating Two patients. Also, the Capitol would like getting its pound of flesh in exchange for the expense of training a quality doctor.
